UCCJEA Affidavit

The UCCJEA Affidavit is a form which must be completed by each party to any case involving the custody of a minor child. The purpose of this form is to disclose vital information to the Court to help determine whether the Court has proper jurisdiction to make decisions concerning the children. It requires the party to list each address for the child for the past five years, including the address, the move in and move out date, and all adults who lived at that address. It also requires a list of any previous relevant cases, child support orders, or custody claims.

This form can be complex and difficult for an individual to complete on their own. To help, Southeastern Ohio Legal Services has created an automated interview. Once you have answered all of the questions, the program will create a word document that you can print, sign, and submit to the Court.

The interview will take approximately 30 minutes to complete. The interview itself is hosted on lawhelpinteractive.org. While registration for that site is optional, we recommend that you register so that you will have the option of saving your answers. This will prevent you from having to start all over if you lose the file with your completed affidavit.
