1. This
application was created using the services provided by Wufoo.com, and is
processed using their servers. The data
transmitted is intended to be private, but is not encrypted. Neither Southeastern Ohio Legal Services
(SEOLS®) nor Ohio State Legal Services Association (OSLSA®)
makes any warranty regarding the confidentiality of this data and does not
accept any responsibility or liability in the unlikely event that the data is
intercepted or otherwise delivered to a third party during transmission.
DO NOT INCLUDE YOUR SOCIAL SECURITY NUMBER IN YOUR APPLICATION.
If you wish to relay information to SEOLS® and
are concerned about the potential security of such data, please call our office

2. By
completing the Application For Services, you are asking SEOLS® to
review your situation as presented, to determine whether our office handles
cases like yours, and to follow up with you by phone or e-mail to obtain more
information. We will not accept a case
for representation based solely upon the information included in this
Application.
SUBMITTING A FORM DOES NOT MAKE YOU A CLIENT OF SOUTHEASTERN OHIO LEGAL SERVICES.
We will treat any information submitted on this form as
information received from a potential client for all purposes including but not
limited to rights of confidentiality and conflict avoidance.
3. Depending
upon the information you submit in this form, we will respond in one of two
ways within 2 business days:
a. If
it is clear from the information you have provided that we cannot assist you
with your problem, we will send you an e-mail indicating that we are not
available in your situation. This can
occur if your information shows that you are seeking help with a problem or in
a legal area that this office does not handle, such as criminal law, worker’s
compensation, or discrimination cases. This will also occur if we are already representing a person whose
interests are in conflict with yours, or if your household income exceeds our
program’s regulations for eligibility.
If you receive an e-mail
indicating that we cannot assist you, and you disagree with that decision, you
should call the office to clarify your request.
b. If,
based on the initial information you have provided, we believe you have a
situation or question that merits further investigation, one of our staff
members will contact you for more detailed information. At that time we will either formally open an
application in your name in our system, or determine that you are not eligible
for our services.
4. You
file will only be reviewed by an attorney after a formal application is taken
pursuant to paragraph 3.b. Once your
file is transferred to an attorney, that attorney will either contact you or
will schedule an in-person interview as soon as practicable.
